Interprofessional Collaborative Practice Models - Local Examples of the Nexus at Work – All are welcome!

Details: Please join us for a panel presentation featuring representatives from local collaborative practice sites in a variety of care settings. This moderated session will highlight how these teams have been formed and how they function. Using the Nexus principles, we will illustrate how similar partnerships can be developed and learners integrated. Featured collaborative practice models will include: the VA Medical Center (iAPACT, VALUE and outpatient mental health programs), Fairview-Riverside Psychiatry, Broadway Family Medicine Clinic, Community-University Health Care Center (CUHCC), the University of Minnesota Dental clinics (Dental and Pharmacy partnership), and a school-based interprofessional collaboration model.
Upon completion of this session participants will be able to:
Identify existing local collaborative practice efforts
Explain the Nexus principles and their importance in developing effective, sustainable clinical interprofessional learning opportunities
Recognize aspects of the work that can be replicated in their own settings
