For Students
Infectious Disease Prevention Educational Module
Section 2: Immunizations and Vaccines

Section 2 Learning Objective
This section is designed to help you:

 - follow the requirements for immunizations and vaccinations.

Immunizations and Vaccines
Boynton Health Service is the place for AHC student immunization data and annual Mantoux testing.  At the Boynton Health Services Immunization Services Web site,  www.bhs.umn.edu/services/immunizationservices.htm, you can find out more about their immunization services, and print a copy of the Student Immunization Record form.

Hardcopy forms are also available at the Boynton Health Service check-in desk.  In addition to your insurance information, you should also carry proof of your immunizations because you may be asked to show them on the first day of your rotation and in emergency situations.

 IMPORTANT!

  • To register for the academic year, you must have the appropriate immunizations.
  • Your failure to have all required immunizations and vaccinations may influence the University's ability to place you in clinical rotations.
  • You must carry documentation of immunizations to early practice/shadowing experience, service-learning and clinical rotations sites.

Additional Information

- Read the AHC Immunization Requirements and Process memo.
- For more information about individual immunizations, visit the CDC National Immunization Program at www.cdc.gov/vaccines/.
